Instructions for applying
(group):
1. Complete EMPLOYER APPLICATION - Part 1 & 2
2. Either complete SPREADSHEET (include employees and dependents who are covered with their associated date of birth and SSN) or paper EMPLOYEE APPLICATIONS
3. Attach a copy of the sold proposal.
4.
Submit a check for the 1st month’s premium made payable to: Standard Life & Accident Insurance Company.

Note: the application for individual accounts is similar but the agency/GA coding is different.
